Attend appointments with clients and make concise case notes to help document vital information for coordination between providers. Provide case notes to other parties if approved by the client
Help organize and manage healthcare records, both written and electronic. Encourage clients to have all necessary Advance Directives in place
Hospital Bedside Advocacy
Be available 24 hours/day for bedside advocacy if client is hospitalized. Not only can this provide comfort and peace of mind for the family members who may be out of the area, but assures adequate and proper treatment of the client for their particular circumstances and diagnoses.
Participate in discharge and transition planning, to include medication reconciliation.
Healthcare Navigation
Help coordinate all aspects of a client’s healthcare journey. From initial medical team contact to resolution or ongoing care. Can help manage transitional planning into retiree healthcare changes and beyond.
